Geriatric Care Managers to become Rainbow Gardens "Stay at Home" Associate Partners within major metropolitan areas of the country. If you are a Registered Nurse (R.N.) with an advanced degree in Social Work, or a similar degree, you may be qualified for this unique career opportunity. (See the Stay at Home Services page of this site for more information about the services offered by the Geriatric Care Manager.) Additional qualifications include:

• Three years experience in geriatric care management
• An R.N. or Community Health Nurse
• Background in social work or similar background in individual care planning
• A small investment is required to become an Associate Partner

The job responsibilities of our professional geriatric care managers include:

Conducting an in-home analysis to develop a "Stay at Home Personal Care Plan" for our resident clients' specific requirements.
Implement personal care plan for client, with recognition of client's cultural differences.
Professional monitoring of subtle changes in client's behavior to avoid crisis incidents.
Home Safety management.
Conflict resolution of complex family issues.
Situation management for out of town families.
Ongoing assessment and intervention measures, and communication with key support systems.
Counseling issues as needed on a separate basis.
Provide select referrals for legal, medical, financial, insurance or accounting needs.
